Background:

Laparoscopic cholecystectomy is the standard for gallstone treatment but is costly and requires specialized training.
Open cholecystectomy remains prevalent in developing regions due to cost and infrastructure limitations.
A modified minimal access approach was developed to address cost and morbidity concerns.

Purpose of the Study:

To evaluate the outcomes of a modified mini-laparoscopic cholecystectomy.
To report on innovations and modifications to the surgical technique.

Main Methods:

A prospective study of 200 patients (aged 15-56) with cholelithiasis from May 2006 to May 2008.
A modified mini-laparoscopic approach using a 3-5cm incision with muscle-cutting/splitting technique.

All procedures were successfully completed with no major complications.
