Acute renal dysfunction (ARD) is a common complication after ventricular assist device (VAD) implantation. Longer bypass time, bleeding, and reoperation increase ARD risk and reduce survival.
Area of Science:
- Cardiology
- Nephrology
- Medical Devices
Background:
- Acute renal dysfunction (ARD) is a frequent complication following ventricular assist device (VAD) implantation.
- Identifying predictors of ARD is crucial for improving patient outcomes after VAD surgery.
Purpose of the Study:
- To identify predictors associated with acute renal dysfunction (ARD) after ventricular assist device (VAD) implantation.
- To analyze the impact of ARD on patient outcomes and survival post-VAD.
Main Methods:
- Patients undergoing VAD implantation were assessed for ARD using the RIFLE criteria.
- A cohort of 24 patients with ARD was compared to 29 patients without ARD (NoARD).
- Pre-VAD characteristics, intraoperative factors, complications, and survival rates were analyzed.
Main Results:
- The ARD group experienced longer cardiopulmonary bypass (CPB) times, increased need for reoperation, and higher intraoperative blood loss.
- Higher incidence of infections, liver injury, ventricular arrhythmias, and right ventricular failure were observed in the ARD group.
- Need for dialysis correlated with elevated pre-VAD creatinine and poorer outcomes, with significantly lower survival in the ARD group.
Conclusions:
- Acute renal dysfunction is a significant complication post-VAD implantation, associated with reduced survival.
- Longer CPB time, substantial intraoperative blood loss, and reoperation are identified as key associated factors for ARD development.
