Summary

Miliary tuberculosis can cause rapid liver and kidney failure. This treatable condition, characterized by acid-fast bacilli, should be considered in patients with unexplained hepatic failure.

Background:

  • Fulminant hepatic failure is a critical condition with high mortality.
  • Disseminated infections can present with multi-organ dysfunction.

Observation:

  • A 67-year-old male presented with acute hepatic and renal failure over six days.
  • Despite intensive supportive care, the patient experienced disseminated intravascular coagulation (DIC) and multi-organ failure (hepatic, renal, respiratory).

Findings:

  • Postmortem examination identified acid-fast bacilli in multiple organ systems.
  • The findings suggest miliary tuberculosis as the underlying cause of the patient's rapid decline.

Implications:

  • Miliary tuberculosis is a critical, yet potentially treatable, cause of acute liver failure.
  • Early consideration of miliary tuberculosis in patients with unexplained hepatic failure may improve outcomes.
