The Thyroid Gland

The thyroid gland is a small, butterfly-shaped gland located in the neck and covers the anterior surface of the trachea. The gland has two lateral lobes connected by a thin tissue mass called the isthmus. Internally, each lobe comprises many small spherical structures known as thyroid follicles, surrounded by a network of blood vessels.

Summary

Branching polycystic and giant thyroglossal duct cysts are rare congenital anomalies. Surgical removal using a hyoid bone-based technique ensures satisfactory outcomes for these challenging thyroglossal duct cyst cases.

Background:

Thyroglossal duct cysts (TDCs) are congenital anomalies arising from remnants of the thyroglossal duct.
Branching polycystic and giant variants of TDCs are exceptionally rare, posing unique diagnostic and therapeutic challenges.

Observation:

This report details two rare cases: a branching, polycystic TDC in an 11-year-old boy and a giant TDC in a 41-year-old man.
Both patients underwent surgical excision utilizing established techniques, including the Sistrunk and Horisawa procedures.

Findings:

Both cases demonstrated successful surgical outcomes with satisfactory post-operative courses.
The management of these rare TDC presentations highlights the importance of tailored surgical approaches.

Implications:

Surgical intervention for anomalous thyroglossal duct cysts should be guided by the specific anatomy of the hyoid bone region.
Effective management of rare TDC subtypes necessitates a thorough understanding of embryological remnants and surgical principles.
