A decade after the Institute of Medicine's report on medical errors, patient safety progress remains slow.
Hierarchical medical structures and a culture discouraging teamwork and transparency hinder safety improvements.
The Lucian Leape Institute was established to guide national patient safety initiatives.

Purpose of the Study:

To introduce five fundamental concepts for improving healthcare system safety.
To outline the implications of these concepts for patient safety.
To set the stage for future discussions on implementing these safety concepts.

Main Methods:

Conceptual framework development by the Lucian Leape Institute.
Identification of five core concepts for healthcare safety.
Illustrative explanation of each concept's meaning and implications.

Main Results:
