Background:

Emergency department (ED) patients frequently need urgent primary care follow-up.
Facilitating timely post-discharge appointments is crucial for effective after-visit care.
Current systems often present barriers to accessing necessary primary care.

Purpose of the Study:

To describe the development and implementation of an appointment assignment system for ED patients.
To evaluate the outcomes of this patient-centered system in improving follow-up care.
To assess the impact on ED recidivism and patient satisfaction.

Main Methods:

Development of a novel appointment assignment system within an integrated healthcare setting.
Implementation of the system across two community hospitals.
Analysis of patient compliance rates and unscheduled ED return visits.

Main Results:

The appointment assignment system achieved a high rate of patient compliance with follow-up appointments.
A very low rate of unscheduled return visits to the ED was observed post-implementation.
The patient-centered approach demonstrated effectiveness in facilitating continuity of care.

Conclusions:

Proactive appointment scheduling from the ED is an effective strategy for ensuring primary care follow-up.
