Area of Science:

  • Orthopedics
  • Biomaterials Science
  • Surgical Technology

Background:

  • Radiofrequency electrocautery is utilized for hemostasis and ablation in arthroscopic shoulder surgery.
  • Previous research has primarily investigated the mechanical properties of sutures against abrasion.
  • The impact of electrical energy on suture integrity during arthroscopic procedures remains a critical concern.

Purpose of the Study:

  • To evaluate the effect of electrical energy from electrocautery on the mechanical properties of five distinct #2 suture materials.
  • To compare the susceptibility of different suture brands to electrocautery-induced damage.
  • To inform surgical techniques and material selection for arthroscopic shoulder repair.

Main Methods:

  • Five brands of #2 suture (FiberWire, Orthocord, Hi-Fi, MaxBraid, Ethibond) were tested.
  • Sutures were submerged in buffered saline solution (pH 7.4) using human deltoid muscle as a medium.
  • Three testing groups were established: control (no current), coagulation (2-second direct current), and cutting (2-second direct current).

Main Results:

  • Hi-Fi suture exhibited the least susceptibility to damage from direct electrocautery application.
  • Orthocord suture demonstrated the greatest reduction in strength among all tested materials.
  • Exposure to electrocautery, both in coagulation and cutting modes, significantly weakened the tested sutures.

Conclusions:

  • Electrocautery use during arthroscopic shoulder repair can compromise suture integrity and strength.
  • Careful application of electrocautery is essential to prevent suture failure and maintain the stability of surgical repairs.
  • Material selection and technique modification may be necessary to mitigate electrocautery-related suture damage.
