Area of Science:

  • Public Health
  • Infectious Diseases
  • Primary Care Medicine

Background:

  • The acquired immunodeficiency syndrome (AIDS) epidemic highlighted the need for sexually transmitted disease (STD) screening.
  • Primary care physicians play a crucial role in identifying and managing STDs.

Purpose of the Study:

  • To assess the extent and determinants of STD screening and counseling by primary care physicians.
  • To identify barriers and facilitators to effective STD prevention in primary care settings.

Main Methods:

  • A 1987 telephone survey was conducted among primary care physicians in Washington, DC, an area with high STD incidence.
  • Ninety-nine physicians (internists, OB/GYNs, family/general practitioners) completed the survey, yielding a 61% response rate.

Main Results:

  • Screening rates varied: 39.4% for gonorrhea, 57.5% for syphilis, and 94% for human immunodeficiency virus (HIV).
  • Hepatitis B screening was positively associated with gonorrhea and syphilis screening.
  • Less than half (45.9%) of physicians inquired about patients' sexual practices.

Conclusions:

  • Physicians should routinely take sexual histories to improve STD detection.
  • Increased preventive counseling regarding sexual practices is essential in primary care.
