Background:

  • Feline chronic progressive polyarthritis (FCPP) is a rare immune-mediated joint disease.
  • FCPP has historically been documented exclusively in male felines.

Observation:

  • A one-year-old female cat presented with anorexia, lassitude, lameness, joint swelling, pain, stiffness, crepitus, regional lymphadenopathy, and fever.
  • Diagnostic workup included imaging, antinuclear antibody testing, synovial fluid analysis, and urinalysis, confirming erosive proliferative periosteal polyarthritis.
  • The cat tested positive for feline leukemia virus antigen and feline immunodeficiency virus antibody.

Findings:

  • Genetic analysis using the sex-determining region on the Y chromosome gene confirmed the cat was genetically female, challenging previous observations.
  • Cyclosporine treatment led to rapid and sustained improvement in the cat's clinical condition and mobility for at least 8 months.

Implications:

  • This case expands the known clinical spectrum of feline chronic progressive polyarthritis to include genetically female cats.
  • The findings suggest that FCPP may occur in both sexes, necessitating broader diagnostic considerations.
  • Successful cyclosporine treatment highlights its potential efficacy in managing FCPP, regardless of genetic sex.
