  • A case study of an 85-year-old Black man with 50-year-old hyperkeratotic, darkly-pigmented papules on sun-exposed skin.
  • Lesions measured 1-2 cm.

Findings:

  • Histopathology revealed large keratinocytes, acanthosis, hypergranulosis, and orthokeratosis.
  • Findings were consistent with a diagnosis of large cell acanthoma.

Implications:

  • Large cell acanthoma should be considered in the differential diagnosis of keratotic or irregularly pigmented lesions.
  • This includes consideration in Black patients, broadening the diagnostic scope for such presentations.
