Primary orbital liposarcoma is rare and challenging to diagnose, often initially misdiagnosed. Exenteration is the preferred treatment, though recurrence can still occur, and it may indicate Li-Fraumeni syndrome.

Area of Science:

  • Ophthalmology
  • Oncology
  • Pathology

Background:

  • Primary orbital liposarcoma is a rare malignancy.
  • Accurate diagnosis and treatment are challenging due to its rarity and potential for misdiagnosis.

Observation:

  • Six new cases of primary orbital liposarcoma were analyzed, including one recurrence.
  • Literature review identified 34 additional cases, with myxoid being the most common subtype (56.8%).
  • Four cases where exenteration was deferred resulted in disease recurrence.

Findings:

  • Exenteration is the treatment of choice, though local recurrence occurred in 2 cases even after exenteration.
  • Well-differentiated liposarcomas demonstrated a better prognosis.
  • Two cases were associated with Li-Fraumeni syndrome and other malignancies.

Implications:

  • Orbital liposarcoma diagnosis and management require specialized expertise.
  • Early recognition of orbital liposarcoma may prompt screening for Li-Fraumeni syndrome.
  • The roles of radiotherapy and chemotherapy in orbital liposarcoma treatment require further investigation.
