Summary

Immediate photorefractive keratectomy (PRK) with mitomycin C (MMC) effectively treated LASIK flap buttonholes. This approach restored 20/20 vision and regular topography in all patients within three months.

Background:

  • Laser-Assisted In Situ Keratomileusis (LASIK) is a common refractive surgery.
  • Complications like LASIK flap buttonholes can occur during the procedure.
  • Prompt management is crucial to ensure optimal visual outcomes.

Observation:

  • Three patients experienced LASIK flap buttonholes in the left eye during bilateral LASIK using the SCHWIND Carriazo-Pendula microkeratome.
  • The buttonhole flaps were repositioned.
  • Phototherapeutic keratectomy (PTK) for epithelial removal was performed, followed by immediate PRK with topical mitomycin C (MMC) 0.02%.

Findings:

  • All three patients achieved uncorrected and corrected distance visual acuity of 20/20.
  • Topographic findings were regular three months post-procedure.
  • The combined PRK and MMC treatment effectively addressed the LASIK flap buttonhole complication.

Implications:

  • Immediate PRK with MMC presents a viable and effective treatment strategy for LASIK flap buttonholes.
  • This technique can restore excellent visual acuity and corneal topography.
  • It offers a solution to mitigate visual impairment from LASIK flap complications.
