Area of Science:

  • Oncology
  • Molecular Biology
  • Genomics

Background:

  • Neoadjuvant chemotherapy improves outcomes for locally advanced esophageal adenocarcinoma.
  • Patient response to neoadjuvant chemotherapy varies significantly.
  • Predictive biomarkers are needed to personalize treatment strategies.

Purpose of the Study:

  • To investigate if pretreatment tumor biopsy gene expression can predict response to neoadjuvant chemotherapy in esophageal adenocarcinoma.
  • To identify specific genes or pathways associated with chemotherapy response.

Main Methods:

  • Microarray analysis of tumor biopsies from 47 patients with locally advanced esophageal adenocarcinoma.
  • Patients received neoadjuvant chemotherapy (cisplatin, 5-fluorouracil, leucovorin) followed by surgery.

  • Statistical comparison of gene expression profiles between responders and non-responders.
  • Immunohistology for Ephrin B3 receptor and Ki-67 was performed.
  • Main Results:

    • 86 genes showed at least 2-fold differential regulation between responding and non-responding tumors.
    • Differentially regulated genes were involved in cell cycle, signal transduction, and cell-cell interactions.
    • Ephrin B3 receptor expression was significantly higher in responders and correlated with chemotherapy response, pathologic staging, and grading.

    Conclusions:

    • Significant differences in gene profiles exist between patients who respond and those who do not respond to neoadjuvant chemotherapy.
    • Customized microarray analysis of pretreatment biopsies may enable prediction of chemotherapy response.
    • This approach could facilitate personalized treatment selection for esophageal adenocarcinoma.
