Motor Units

A motor unit consists of two main components: a single efferent motor neuron (i.e., a neuron that carries impulses away from the central nervous system) and all of the muscle fibers it innervates. The motor neuron may innervate multiple muscle fibers, which are single cells, but only one motor neuron innervates a single muscle fiber.

Background:

Obtaining informed consent for procedures in intensive care units (ICUs) is complex.
Practices for consent vary significantly across academic medical centers.
Standardized consent processes may improve patient care and adherence.

Purpose of the Study:

To assess national practices for obtaining consent for routine bedside procedures in academic adult ICUs.
To determine universal consent rates based on patient demographics within a specific institutional setting.

Main Methods:

A 10-question survey was distributed to U.S. surgical and pulmonary critical care program directors to evaluate consent practices.
A universal consent protocol was implemented and studied in an academic county hospital's ICUs.

Main Results:

Thirty-seven percent of program directors responded to the survey, revealing consent rates ranging from 35% to 97% by procedure.
Medical ICUs obtained consent more frequently than Surgical ICUs for central line and pulmonary artery catheter placement.
At the study institution, 90% of patients or proxies signed universal consent, with insured patients being more likely to provide full consent.

Conclusions:

Significant national variation exists in ICU consent practices, with a recognized interest in broader adoption of universal consent protocols.
Universal consent adoption was influenced by patient demographics, indicating potential disparities.
The universal consent protocol was well-received and widely accepted within the studied institution.
