Summary

Transpupillary thermotherapy (TTT) effectively reduced intraocular pressure (IOP) in patients with Sturge-Weber syndrome (SWS) and late-onset juvenile glaucoma (LOJG). This treatment for diffuse choroidal haemangioma (DCH) also prevented complications.

Background:

  • Sturge-Weber syndrome (SWS) can lead to late-onset juvenile glaucoma (LOJG) and diffuse choroidal haemangioma (DCH).
  • Uncontrolled intraocular pressure (IOP) in LOJG poses a risk of vision loss.

Observation:

  • Two patients with SWS, DCH, and uncontrolled LOJG underwent transpupillary thermotherapy (TTT).
  • TTT involved selective, partial destruction of DCH using an 810 nm infrared diode laser in multiple sessions.
  • Laser parameters were adjusted to achieve tumor discoloration without causing complications.

Findings:

  • TTT resulted in normalization of IOP in both patients.
  • The progression of LOJG was halted during follow-up periods of up to 6 years.
  • No visual loss or other complications were observed post-treatment.

Implications:

  • TTT is a viable therapeutic option for managing both DCH and LOJG in SWS patients.
  • This approach may prevent exudative retinal detachment associated with DCH.
  • TTT offers a single-modality treatment for complex ocular conditions in SWS.
