Abstract:
Infective endocarditis is one of the uncommon causes of complete atrioventricular (AV) block. Complications from aortic valve infection are more often associated with conduction abnormalities than with mitral valve endocarditis. In fact, complete AV block rarely occurs in mitral valve infections, with only a few cases having been reported. We report a case of mitral valve endocarditis in which the initial presentation was fever and newly developed, complete AV block. Patients with mitral valve endocarditis complicated by complete AV block are at high risk. Prompt investigation, administration of appropriate antibiotics and consideration of early surgical intervention are needed.
Insights
Infective endocarditis can rarely cause complete atrioventricular (AV) block, particularly with mitral valve infections. This case highlights the high risk and need for prompt treatment in such rare instances.
Area of Science:
- Cardiology
- Infectious Diseases
- Cardiac Electrophysiology
Background:
- Infective endocarditis is a complex infection affecting heart valves.
- Complete atrioventricular (AV) block is an uncommon but serious complication.
- Aortic valve endocarditis more frequently causes conduction abnormalities than mitral valve endocarditis.
Observation:
- This report details a rare case of mitral valve endocarditis presenting with fever and new-onset complete AV block.
- Complete AV block is exceptionally rare in the context of mitral valve infections, with few cases documented.
- The patient's presentation underscores the potential for severe cardiac complications from mitral valve endocarditis.
Findings:
- Mitral valve endocarditis can manifest as complete atrioventricular (AV) block.
- Patients experiencing this combination face significant risks.
- Early diagnosis and intervention are crucial for managing this condition.
Implications:
- This case emphasizes the importance of considering infective endocarditis in patients presenting with unexplained complete AV block, even with mitral valve involvement.
- Prompt investigation, including echocardiography and blood cultures, is essential.
- Aggressive management, including appropriate antibiotic therapy and consideration for early surgical valve replacement, is critical for improving patient outcomes and preventing mortality.
