Summary

Surgical treatment of C2 dens fractures with direct osteosynthesis showed better outcomes than conservative management. Surgery resulted in improved range of motion and reduced pain scores compared to non-operative methods.

Main Results:

  • The surgical group demonstrated superior results in range of motion (ROM), Visual Analogue Scale (VAS) pain scores, and radiographic examinations compared to the conservative group.
  • Only one patient in the surgical group experienced restricted cervical ROM, versus three in the conservative group.
  • No fragment displacement or pseudojoint formation occurred in the surgical group, while two cases of fragment displacement were noted in the conservative group (both healed).

Conclusions:

  • Direct osteosynthesis for C2 dens fractures offers better functional outcomes and lower complication rates than conservative treatment.
  • Surgical management with canalised screws and the Dens Access device appears to be an effective treatment for C2 dens fractures.
