Background:

  • Takayasu's arteritis (TA) is a rare, chronic inflammatory disease affecting large arteries.
  • Limited data exists on the efficacy of mycophenolate mofetil (MMF) in managing TA, particularly in Asian Indian populations.
  • Existing treatments often involve corticosteroids, which can lead to significant side effects.

Purpose of the Study:

  • To evaluate the safety and efficacy of oral mycophenolate mofetil (MMF) as an immunosuppressive therapy in Asian Indian patients diagnosed with Takayasu's arteritis (TA).
  • To assess the impact of MMF on disease activity, inflammatory markers, and corticosteroid dosage in TA patients.

Main Methods:

  • A retrospective study of 21 consecutive Asian Indian patients with TA treated with oral MMF from January 2005 to August 2008.
  • Disease activity was assessed using the Indian Takayasu's arteritis activity score (ITAS) and physician's global observation score at baseline and follow-up.
  • Changes in laboratory markers of inflammation (ESR, CRP) and corticosteroid dosage were analyzed.

Main Results:

  • Nineteen patients (90%) received MMF for active disease; one patient discontinued due to rash.
  • All 20 patients continuing MMF showed significant improvement in disease activity (median ITAS dropped from 7 to 1, p=0.001).
  • Significant reductions were observed in Erythrocyte Sedimentation Rate (ESR) and C-Reactive Protein (CRP) levels, along with a decrease in mean daily steroid dosage (36 mg to 19 mg, p<0.001).

Conclusions:

  • Mycophenolate mofetil (MMF) is a safe and effective steroid-sparing immunosuppressant for managing Takayasu's arteritis (TA) in Asian Indian patients.
  • MMF demonstrates significant efficacy in reducing disease activity and inflammatory markers, enabling a reduction in corticosteroid dependence.
  • This study represents the largest series to date supporting the use of MMF in TA treatment.
