Summary

Chinese immigrants from Southeast Asia have a threefold higher risk of common bile duct stones (choledocholithiasis) during gallbladder removal surgery. Intraoperative cholangiography is recommended for all such patients to ensure early detection and management.

Area of Science:

  • Gastroenterology
  • Hepatobiliary Surgery
  • Epidemiology

Background:

  • Biliary tract disease admissions were evenly split between Southeast Asian Chinese immigrants and other ethnic groups.
  • Cholelithiasis (gallstones) is a common biliary tract disease.

Purpose of the Study:

  • To investigate the incidence and severity of choledocholithiasis in Southeast Asian Chinese immigrants undergoing cholecystectomy.
  • To determine if ethnic background influences the risk of common bile duct stones.

Main Methods:

  • Retrospective analysis of patient admissions for biliary tract disease from January 1987 to December 1989.
  • Comparison of choledocholithiasis incidence between Southeast Asian Chinese immigrants and other ethnic groups undergoing cholecystectomy for cholelithiasis.

Related Experiment Videos

Main Results:

  • The incidence of choledocholithiasis was significantly higher in Chinese immigrants (37.2%) compared to other groups (11.8%).
  • Chinese patients were more likely to present with severe disease, including multiple ductal stones and significant ductal enlargement.
  • This increased risk was consistent across different age groups.

Conclusions:

  • Southeast Asian Chinese immigrants face a substantially elevated risk of choledocholithiasis when undergoing surgery for gallstones.
  • Intraoperative cholangiography is a mandatory procedure for all Chinese immigrants from Southeast Asia undergoing cholecystectomy, irrespective of other indications for common bile duct exploration.
