Background:

  • Intravascular menstrual endometrium is an uncommon finding in hysterectomy specimens.
  • It can present diagnostic challenges, particularly in differentiating it from malignant conditions.

Observation:

  • A case of a 28-year-old female with cervical dysplasia is presented.
  • Numerous foci of menstrual endometrium were identified within the parametrial blood vessels of her hysterectomy specimen.

Findings:

  • Histological examination revealed intravascular tissue composed of spindled stromal cells (vimentin positive) and cuboidal epithelial cells (epithelial marker positive).
  • The appearance was cytologically bland and biphasic.
  • Immunohistochemistry confirmed the endometrial origin of the intravascular components.
