Summary

This study found that oblong acetabular implants, particularly those with hydroxyapatite coating, offer improved survival rates for acetabular defects compared to their original models. Proper anchoring in load-bearing bone is crucial for success.

Area of Science:

  • Orthopedic surgery
  • Biomaterials science
  • Medical device engineering

Background:

  • Reconstructing defective acetabular bone beds is essential for successful hip implant surgery.
  • Oblong implants are an option for acetabular reconstruction, but data are limited.
  • This study compares oblong implant outcomes with their original models.

Purpose of the Study:

  • To evaluate the efficacy and survival rates of oblong acetabular implants.
  • To compare the outcomes of the Langs-Ovale Revisionspfanne (L.O.R.) with the Walter Oval cup.
  • To investigate the role of hydroxyapatite coating in implant survival.

Main Methods:

  • Comparative study of 111 patients (L.O.R.) and 134 patients (Walter Oval) undergoing acetabular revision.
  • Kaplan-Meier survival analysis and Harris Hip Score (HHS) evaluation.

  • Radiographic assessment for implant migration and loosening.
  • Main Results:

    • The Walter Oval group had a lower revision rate (5.3%) compared to the L.O.R. group (13.5%).
    • Aseptic loosening and deep infection were noted as causes for revision.
    • Both implant types showed significant improvement in HHS, with no radiographic differences observed.

    Conclusions:

    • Oblong implants are favorable for acetabular defects (Paprosky types IIB-IIIA).
    • Successful outcomes depend on anchoring the oblong cup in the patient's own bone.
    • Hydroxyapatite coating significantly enhances implant survival compared to the original model.
