Summary

Hypertension and its treatments can impair penile arterial function, affecting erectile response. Certain blood pressure medications, particularly thiazides, may worsen arterial function more than hypertension duration itself.

Area of Science:

  • Urology
  • Cardiovascular Medicine
  • Pharmacology

Background:

  • Hypertension is a significant risk factor for erectile dysfunction (ED).
  • The impact of antihypertensive medications on penile vascular function requires further elucidation.
  • Pharmacologic duplex sonography offers objective assessment of penile hemodynamics.

Purpose of the Study:

  • To evaluate the effects of hypertension and its treatment on penile arterial function in impotent patients.
  • To compare the impact of different antihypertensive drug classes on penile hemodynamics.
  • To assess the correlation between arterial response and erectile function.

Main Methods:

  • Pharmacologic duplex sonography with papaverine injection in 472 impotent patients.
  • Assessment of penile arterial pulsation, diameter, and blood flow velocity.
  • Analysis of patient history including hypertension duration, comorbidities, and medication use.

Main Results:

  • 85% of patients exhibited impaired penile arterial function.
  • Hypertension duration had less impact than diabetes or smoking.
  • Patients on antihypertensive medication showed poorer arterial response compared to non-medicated patients.
  • Thiazide diuretics appeared to have a detrimental effect on arterial function.

Conclusions:

  • Hypertension and certain antihypertensive drugs negatively impact penile arterial function, contributing to erectile dysfunction.
  • Antihypertensive medication choice may influence erectile function beyond blood pressure control.
  • Functional venous leak may result from altered erectile tissue compliance due to specific medications.
