Background:

  • Idiopathic generalized epilepsies (IGEs) can present with focal discharges on electroencephalograms (EEGs).
  • The origin of these focal discharges in IGEs is not fully understood.
  • Subtle structural brain abnormalities may underlie these EEG findings.

Purpose of the Study:

  • To investigate the relationship between focal epileptiform discharges on EEG and subtle structural magnetic resonance imaging (MRI) abnormalities in patients with IGEs.
  • To assess if focal discharges originate from areas of structural abnormality detectable by quantitative neuroimaging.

Main Methods:

  • Quantitative EEG source detection was used to localize focal discharges.
  • Voxel-based morphometry (VBM) and region of interest (ROI) volumetry were employed for neuroimaging analysis.
  • Individual patient MRI scans were compared to a control group to identify gray matter volume differences.

Main Results:

  • Voxel-based morphometry analysis detected gray matter abnormalities in 77% of the 22 evaluated IGE patients.
  • A good concordance was observed between EEG source localization and VBM findings.
  • The average distance between detected abnormalities was 19 mm, with the most significant at 34 mm.

Conclusions:

  • Subtle gray matter abnormalities are associated with focal epileptiform discharges in some patients with IGEs.
  • Quantitative neuroimaging can detect structural abnormalities related to focal EEG findings.
  • This study provides evidence linking EEG abnormalities to underlying structural changes in IGEs.
