Summary

This study on uterine sarcoma found that early stage and low grade tumors significantly improve survival. Adjuvant pelvic radiotherapy after surgery also enhances 5-year disease-free survival (DFS) in uterine sarcoma patients.

Area of Science:

  • Gynecologic Oncology
  • Surgical Oncology
  • Radiation Oncology

Background:

  • Uterine sarcoma is a rare and aggressive malignancy.
  • Understanding prognostic factors and treatment outcomes is crucial for improving patient survival.

Purpose of the Study:

  • To analyze the clinical characteristics, failure patterns, and survival rates of patients diagnosed with uterine sarcoma.
  • To identify key predictors of survival and the impact of adjuvant therapies.

Main Methods:

  • Retrospective review of 23 uterine sarcoma patient records from July 1993 to May 2007.
  • Data collected included patient demographics, histological subtypes, treatment modalities (surgery, radiotherapy, chemotherapy), failure patterns, and survival outcomes.
  • Histological confirmation was required for all cases.

Main Results:

  • The majority of patients (65.2%) had endometrial stromal sarcoma, with most (65.2%) diagnosed at Stage I.
  • High tumor grade was observed in 52.2% of patients.
  • Five-year disease-free survival (DFS) was 59.6% overall. Patients receiving surgery plus radiotherapy had an 87.5% 5-year DFS compared to 36.4% for surgery alone.
  • Early-stage disease (Stage I) and low-grade tumors were associated with significantly better 5-year DFS (74% and 100%, respectively).

Conclusions:

  • Tumor stage and grade are critical prognostic indicators for uterine sarcoma survival.
  • Adjuvant pelvic radiotherapy following surgery is associated with improved 5-year disease-free survival in uterine sarcoma patients.
