  • To emphasize the importance of considering rare diagnoses in athletes with exercise-induced symptoms.

  • Case report of a 41-year-old ultramarathon runner.
  • Clinical presentation of exercise-related chest pain radiating to the left arm.
  • Diagnostic workup including extensive testing.
