Summary

Hydrochlorothiazide-amiloride can cause acute pulmonary edema, a rare but serious side effect. This noncardiogenic reaction, possibly due to increased vascular permeability, requires clinical attention.

Background:

  • Hydrochlorothiazide-amiloride is a commonly prescribed diuretic.
  • Pulmonary edema is a potential adverse effect of this medication.

Observation:

  • A 44-year-old woman experienced acute pulmonary edema 30 minutes after ingesting hydrochlorothiazide-amiloride.
  • Eighteen prior cases of similar reactions were identified.

Findings:

  • The pulmonary edema appears to be noncardiogenic in origin.
  • Increased pulmonary vascular permeability is a suspected mechanism, though immunological causes have not been confirmed.
  • The pathophysiology remains uncertain.

Implications:

  • This rare side effect can mimic left ventricular failure, complicating diagnosis in cardiac patients.
