Peripheral sympathetic nervous system (pSNS) dysfunction is prevalent in chronic complex regional pain syndrome type I (CRPS I) patients, impacting hand function. This study found no correlation between pSNS function and clinical outcomes in CRPS I.

Background:

  • Sympathetic nervous system (SNS) dysfunction is frequently observed in early stages of complex regional pain syndrome type I (CRPS I).
  • Chronic CRPS I can lead to persistent pain and impaired hand function.

Purpose of the Study:

  • To evaluate peripheral sympathetic nervous system (pSNS) function in patients with chronic CRPS I.
  • To correlate pSNS function with hand function measurements in these patients.

Main Methods:

  • Thirty-two patients with a diagnosis of CRPS I of the upper extremity (duration ≥ 3 years) were assessed.
  • Hand function was evaluated using the Michigan Hand Outcome Questionnaire (MHQ).
  • Peripheral sympathetic nervous system function was tested using laser Doppler flowmetry (LDF).

Main Results:

  • Mean time since diagnosis was 4.6 years.
  • Patients showed significantly lower MHQ scores for the affected hand compared to the unaffected hand (P=0.002).
  • 62.5% of patients exhibited abnormal pSNS function, with a mean sympathetic reflex of 0.18 (normal >0.38).

Conclusions:

  • Significant pSNS impairments persist in nearly two-thirds of CRPS I patients even 5 years post-diagnosis.
  • Patients experience ongoing pain and reduced hand function compared to their unaffected limb.
  • No statistically significant correlation was found between pSNS function and clinical outcomes (MHQ scores) in this cohort.
