Summary

Orbital hyalinizing spindle cell tumor with giant rosettes (HSCTGR) is a rare orbital tumor. Complete excision with close follow-up is the recommended treatment for this condition.

Background:

  • Orbital tumors can present with proptosis and vision changes.
  • Hyalinizing spindle cell tumor with giant rosettes (HSCTGR) is a rare mesenchymal neoplasm.
  • Orbital involvement of HSCTGR has not been previously reported.

Observation:

  • Two pediatric patients presented with proptosis and vision impairment.
  • CT imaging revealed orbital masses with varying extent.
  • Histopathology confirmed HSCTGR with characteristic spindle cells, giant rosettes, and hyalinized stroma.

Findings:

  • Tumor cells were positive for vimentin, with focal S100 positivity in one case.
  • The second case exhibited a higher Ki-67 index, indicating higher proliferative activity.
  • Both patients achieved complete excision without recurrence or metastasis at 12-month follow-up.

Implications:

  • HSCTGR should be considered in the differential diagnosis of orbital fibrous tumors.
  • Early diagnosis and complete surgical excision are crucial for favorable outcomes.
  • This study highlights the potential for HSCTGR to occur in pediatric orbital tissues.
