Summary

Two rare cases of de novo brain aneurysms highlight the need for comprehensive screening. Patients with initial aneurysms require follow-up imaging, including the posterior circulation, to detect new aneurysm formation.

Background:

  • Cerebral aneurysms are abnormal bulges in brain blood vessels.
  • De novo aneurysm formation, especially in the posterior circulation, is uncommon.
  • Previous aneurysm treatment, such as clipping, may not preclude new aneurysm development.

Observation:

  • Two patients developed de novo aneurysms in the basilar artery region.
  • The first patient had a basilar tip aneurysm 5 years after middle cerebral artery aneurysm clipping.
  • The second patient developed a basilar artery-superior cerebellar artery aneurysm 3 years after A(1) aneurysm clipping.

Findings:

  • De novo aneurysm formation occurred in the posterior circulation in both cases.
  • Aneurysms developed years after initial treatment of aneurysms in different locations.
  • These cases demonstrate the potential for new aneurysm development at previously unaffected sites.

Implications:

  • Screening for de novo aneurysms is crucial, particularly in the posterior circulation.
  • Post-treatment surveillance should extend beyond the initial aneurysm site.
  • Comprehensive neurovascular imaging protocols are essential for long-term patient management.
