Summary

Laparoscopic nephrectomy with retroperitoneoscopic access successfully treated renal vein and vena cava thrombus in a clear cell cancer patient. This minimally invasive approach offers a viable option for complex renal tumor cases.

Background:

  • Laparoscopic nephrectomy is increasingly reported for complex cases.
  • Preoperative diagnosis of venous system involvement in renal tumors is becoming more common.

Observation:

  • A case of laparoscopic nephrectomy using retroperitoneoscopic access is presented.
  • The patient had a preoperatively diagnosed renal vein and vena cava thrombus.

Findings:

  • The procedure took 130 minutes with 50 mL blood loss.
  • Pathology confirmed pT3b clear cell cancer (Fuhrman grade 2) with negative venous margins.

Implications:

  • Retroperitoneoscopic laparoscopic nephrectomy is a feasible approach for renal tumors with venous thrombus.
  • Minimally invasive surgery can achieve oncological safety in complex renal cancer cases.
