Summary

Pediatric ovarian masses are concerning for malignancy, especially in young children (1-8 years). Key indicators include a palpable mass, precocious puberty, large size (>8 cm), or solid imaging features, aiding in risk stratification.

Area of Science:

  • Pediatric Oncology
  • Gynecologic Oncology
  • Diagnostic Imaging

Background:

  • Ovarian masses in children have a 10% malignancy rate.
  • Accurate preoperative risk stratification is crucial for appropriate management.

Purpose of the Study:

  • To identify preoperative factors that distinguish high-risk pediatric ovarian masses.
  • To aid in risk stratification for pediatric ovarian masses.

Main Methods:

  • Retrospective review of 424 pediatric ovarian mass cases over 15.5 years.
  • Analysis of patient demographics, presenting symptoms, imaging characteristics, and tumor markers.

Main Results:

  • Malignancies were more common in patients aged 1-8 years (22%) and associated with symptoms of mass or precocious puberty.

  • Ovarian masses >8 cm or with solid imaging features had significantly higher odds of malignancy (OR 19.0).
  • Elevated beta-human chorionic gonadotropin (beta-HCG), alpha fetoprotein (alphaFP), and cancer antigen 125 (CA-125) were associated with malignancy.
  • Conclusions:

    • Preoperative indicators for pediatric ovarian malignancy include mass presentation, precocious puberty, size >8 cm, and solid imaging characteristics.
    • Younger age (1-8 years) is associated with a higher incidence of malignancy.
    • Tumor markers can be useful but are not conclusive for all cases.
