Background:

  • Malignant struma ovarii (MSO) is a rare ovarian teratoma containing thyroid tissue with malignant potential.
  • Primary MSO, originating in the ovary, is exceptionally uncommon, posing diagnostic and therapeutic challenges.

Observation:

  • A 40-year-old woman presented with pelvic pain and a palpable pelvic mass.
  • Initial investigations including tumor markers and biochemistry were unremarkable.
  • Gynecological examination revealed a large pelvic mass.

Findings:

  • Post-operative histopathology confirmed primary malignant struma ovarii.
  • The patient underwent total abdominal hysterectomy and bilateral salpingo-oopherectomy, followed by retroperitoneal pelvic lymphadenectomy for staging.
  • Despite normal findings in the left ovary and lymph nodes, thyroidectomy was declined by the patient.
