Background:

  • Cyclic thrombocytopenia is a rare disorder with unpredictable, severe bleeding episodes.
  • Current treatments for cyclic thrombocytopenia have yielded disappointing results.
  • The pathophysiology of cyclic thrombocytopenia remains largely unknown.

Observation:

  • A patient with a 7-year history of cyclic thrombocytopenia, refractory to multiple treatments, experienced oscillations in platelet counts.
  • Platelet-associated antibodies were positive, and platelet survival time was shortened.
  • Danazol therapy was initiated for the patient's refractory cyclic thrombocytopenia.

Findings:

  • Danazol therapy led to significantly higher nadir platelet counts within 2 months.
  • Petechiae resolved within 9 months of danazol treatment.
