  • Major depressive disorder (MDD) is associated with an elevated risk of sudden cardiac death.
  • Impaired arterial baroreflex function is linked to ventricular arrhythmias.

Purpose of the Study:

  • To investigate arterial baroreflex dysfunction in MDD patients.
  • To determine if electroconvulsive therapy (ECT) improves depressive symptoms and baroreflex function.

Main Methods:

  • Thirty-three hospitalized MDD patients were assessed before and after ECT.
  • Arterial baroreflex sensitivity (BRS) and effectiveness were measured using continuous ECG and blood pressure monitoring.
  • Twenty healthy subjects served as controls.

Main Results:

  • MDD patients exhibited elevated heart rate and blood pressure, with reduced BRS and baroreflex effectiveness compared to controls.
  • ECT reduced depressive symptoms but did not alter cardiovascular or baroreflex measures.
  • These cardiovascular and baroreflex parameters remained unchanged at 6-month follow-up.

Conclusions:

  • Arterial baroreflex sensitivity and activity are reduced in MDD.
  • Baroreflex dysfunction may persist long-term, even after depressive symptoms improve with ECT.
