Background:

Eccrine hidrocystomas are common benign cystic growths.
Typically, they measure 1-5 mm and appear on the eyelids, not near the lid margin.

Observation:

A unique case of a very large eccrine hidrocystoma on the eyelid was observed.
This specific hidrocystoma exhibited a previously undescribed pedunculated morphology.

Findings:

The study details the pathological features of this large, pedunculated eccrine hidrocystoma.
Differential diagnoses for various cystic eyelid lesions are explored.

Implications:

This case expands the known morphological spectrum of eccrine hidrocystomas.
It highlights the importance of considering unusual presentations in the diagnosis of eyelid cystic lesions.
