Communication in conversation in stroke patients

Stroke patients experience complex communication issues beyond simple language deficits. This study reveals that both verbal and non-verbal communication are affected, with specific patterns depending on lesion location.

- Sixty-three patients with various lesion types (left/right cortico-sub-cortical, sub-cortical, frontal, posterior fossa) were assessed.
- A control group matched for age, gender, and education was used for comparison.
- The Lille Communication Test, Functional Communication Profile, and Boston Diagnostic Aphasia Examination were administered.
Main Results:
- Reduced communication participation was observed in left/right cortico-sub-cortical and frontal lesion patients.
- Verbal communication was significantly impaired in left cortico-sub-cortical and left sub-cortical groups, including verbal pragmatic deficits.
- Non-verbal communication was primarily affected in right cortico-sub-cortical patients, with notable pragmatic difficulties. Left cortico-sub-cortical patients showed increased gesture production as a compensatory mechanism.
Conclusions:
- Communication disorders post-stroke are complex, not solely attributable to lexical/syntactic deficits in left lesions or pragmatic issues in right lesions.
- Left-hemisphere stroke patients often exhibit severe verbal pragmatic difficulties.
- Frontal lobe strokes lead to significant verbal and non-verbal communication impairments.
