Summary

This case report details a rare primary peritoneal spindle cell sarcoma presenting with massive ascites. Chemotherapy with doxorubicin and ifosfamide led to symptom improvement and ascites regression.

  • Soft tissue sarcomas (STSs) are rare neoplasms originating from mesenchymal cells.
  • Primary peritoneal sarcomas, including leiomyosarcoma, liposarcoma, and carcinosarcoma, have been documented.

Observation:

  • A 38-year-old woman presented with massive ascites.
  • The diagnosis was primary peritoneal spindle cell sarcoma, a type previously unreported.

Findings:

  • The patient received doxorubicin and ifosfamide chemotherapy.
  • Symptoms improved, and ascites regressed following treatment.

Implications:

  • Primary peritoneal spindle cell sarcoma is a rare but significant cause of massive ascites.
  • Consider primary peritoneal sarcomas in the differential diagnosis for malignancy-associated massive exudative ascites.
