Severe urinary symptoms during prostate cancer radiotherapy can indicate bladder stones (urolithiasis). Early diagnosis and treatment, such as cystolitholapaxy, are crucial for symptom resolution in patients undergoing radiation therapy.

Observation:
- A patient undergoing salvage radiotherapy for prostate cancer developed severe, culture-negative urinary symptoms early in treatment.
- Symptoms included hematuria, dysuria, and passage of cellular debris, unresponsive to radiation therapy breaks.
Findings:
- Imaging revealed a 3.5 cm bladder stone (urolith).
- Urinary symptoms resolved completely after laser cystoscopy and cystolitholapaxy (stone removal).
Implications:
- Urolithiasis is a rare but important consideration in patients experiencing severe, early, culture-negative dysuria during prostate cancer radiotherapy.
- Prompt identification and management of bladder stones can alleviate radiotherapy-induced urinary complications.
