Summary

Laparoscopic sphincter-preserving total mesorectal excision (TME) for rectal cancer demonstrates safe, long-term oncological outcomes comparable to open surgery. This minimally invasive approach offers comparable survival and disease-free survival rates.

Study period: March 1999 to March 2009.

Median follow-up of 49 months.

Main Results:

Low conversion rate (1%) and no operative mortality.
Local recurrence observed in 5.1% of patients.
Overall 5-year survival: 74%; 5-year disease-free survival: 71%.

Conclusions:

Laparoscopic sphincter-preserving TME is a safe procedure for rectal cancer.
Long-term oncological outcomes are comparable to open TME.
Minimally invasive approach provides effective oncological control.
