Summary

Salvage ankle arthrodesis using blade plates shows success after total ankle arthroplasty failure. Other fixation methods in inflammatory joint disease lead to high nonunion rates, impacting outcomes.

The ankle is formed by the talocrural joint (crural = leg). It consists of the articulations between the talus bone of the foot and the distal ends of the tibia and fibula of the leg. Purpose of the Study:

  • Evaluate clinical, radiographic, and subjective outcomes of salvage ankle arthrodesis.
  • Assess the influence of fixation method on union rates.
  • Investigate outcomes specifically in inflammatory joint disease (IJD).

Main Methods:

  • Retrospective analysis of 18 ankles (18 patients) undergoing salvage arthrodesis (1994-2005).
  • Diagnoses included 15 IJD and 3 osteoarthritis (OA).
  • Fusion types: tibio-talar (7) and tibio-talocalcaneal (11). Fixation methods varied; blade plates used in 7 cases.

Main Results:

  • All 7 ankles treated with blade plates achieved union (4 IJD, 3 OA).
  • 7 of 11 IJD ankles treated with other methods developed nonunion.
  • Clinical outcomes (AOFAS, FFI, VAS) were fair to good, irrespective of union status in the evaluated subgroup.

Conclusions:

  • Blade plate fixation is a successful method for salvage ankle arthrodesis post-TAA failure.
  • High nonunion rates are associated with alternative fixation methods in IJD.
  • Salvage arthrodesis offers fair to good clinical results, with fixation choice being critical.
