Branchial fistula--a review.

Background:

Congenital branchial fistulas are well-documented, with embryological origins typically linked to the second and third branchial arches.
The standard surgical treatment for extensive fistulas is the stepladder excision technique.

Observation:

This paper reports three cases of congenital branchial fistulas.
Cases 1 and 2 exhibited typical second and third arch origins, respectively.
Case 3 presented a unique, previously undescribed combination of first and second branchial arch origins.

Findings:

Cases 1 and 2 were treated using the conventional stepladder excision method.
Case 3 was treated with a novel approach: stripping the fistulous tract using a Myer's vein stripper.
This new method offers simplicity and reduced dissection time compared to the stepladder technique.

Implications:

The vein stripper method provides a less invasive alternative for specific congenital branchial fistula cases.
Further research is needed to establish the safety and efficacy of this blind procedure, especially when vital structures are nearby.
This case report expands the understanding of branchial fistula embryology and surgical management options.
