Accessory Structures of the Skin: Nails

Nails are one of the important accessory structures of the skin. They are hard, protective structures that cover the dorsal surface of the distal phalanges of fingers and toes. Nails are composed of specialized keratinized cells and serve various functions, including protection, sensation, and manual dexterity.
The main components of a nail include the following.
Nail Plate: The nail plate is the visible portion of the nail that extends beyond the fingertips or toes. It is a hard, translucent...

Diabetic Foot Ulcer

Definition A diabetic foot ulcer (DFU) is a chronic, non-healing wound that develops in individuals with diabetes. It typically occurs on pressure-bearing areas such as the heel, metatarsal heads, or hallux, and carries a high risk of infection and amputation.Pathophysiology • The development of DFUs can be explained by four interconnected mechanisms: neuropathy, ischemia, infection, and impaired wound healing. • Neuropathy is the most common factor. Sensory neuropathy reduces pain perception,...

Area of Science:

  • Podiatry
  • Dermatology
  • Surgical Techniques

Background:

  • Nail disorders are common in the elderly, with repeated avulsion procedures causing complications.
  • This study addresses the need for effective permanent nail ablation techniques.

Purpose of the Study:

  • To evaluate the effectiveness of a phenol-based total matricectomy technique for permanent nail ablation.
  • To assess patient pain levels and satisfaction post-procedure.

Main Methods:

  • Total nail ablation was performed on 34 hallux toenails (30 patients) using phenol and gauze application.
  • Postoperative pain and patient satisfaction with cosmetic appearance and the procedure were evaluated at 12 months.

Main Results:

  • Preoperative severe pain was reported by all patients; 94.11% experienced no pain 12 months post-surgery.
  • 82.35% of patients were pleased with the cosmetic outcome.
  • 100% of patients reported high satisfaction with the overall procedure.

Conclusions:

  • Phenol-based total nail ablation using gauze is a safe, cost-effective method.
  • The technique yields high patient satisfaction and effective pain relief.
  • Limitations include a small study cohort over an extended period.
