Inhaled compressed gas cleaner caused severe lung injury. Nebulized prostacyclin during high-frequency percussive ventilation significantly improved oxygenation, enabling patient evacuation.

- High-frequency percussive ventilation (HFPV) combined with nebulized prostacyclin therapy significantly improved the arterial oxygen tension/fraction of inspired oxygen ratio from 57 to 147 mm Hg.
- This improvement facilitated reduced ventilator support and safe patient transfer.
Implications:
- Nebulized prostacyclin may be a valuable adjunctive therapy for specific ARDS etiologies.
- Further research is needed to confirm the efficacy of inhaled prostacyclin in HFPV for ARDS.
