Summary

Acupuncture shows promise for primary dysmenorrhoea pain relief compared to conventional treatments. However, methodological flaws in studies limit definitive conclusions, especially when compared to sham acupuncture.

Background:

Effectiveness of acupuncture for primary dysmenorrhoea remains unclear.
Primary dysmenorrhoea significantly impacts women's quality of life.

Purpose of the Study:

To systematically review randomized controlled trials (RCTs) assessing acupuncture for primary dysmenorrhoea.
Evaluate acupuncture's efficacy in symptomatic treatment of menstrual pain.

Main Methods:

Systematic search of 19 electronic databases up to July 2008.
Included RCTs comparing acupuncture to controls; excluded moxibustion and acupressure.
Study abstraction and quality assessment followed Cochrane Handbook guidelines.

Main Results:

Twenty-seven RCTs were reviewed; nine clearly described randomization, none reported allocation concealment.
Acupuncture showed significant pain reduction versus pharmacological or herbal treatments.
Two RCTs found no significant difference between acupuncture and sham acupuncture.

Conclusions:

Promising evidence supports acupuncture for primary dysmenorrhoea compared to conventional medicine.
Methodological limitations temper the strength of the evidence.
Further rigorous placebo-controlled RCTs are needed for convincing results against sham acupuncture.
