Sentinel node navigation using a dual tracer technique during laparoscopic surgery for gastric cancer is feasible. This method aids in identifying micrometastases in sentinel lymph nodes, improving cancer staging.

Area of Science:

  • Surgical Oncology
  • Minimally Invasive Surgery
  • Gastrointestinal Cancer Research

Background:

  • Sentinel lymph node (SLN) navigation is increasingly applied to gastric cancer.
  • SLN identification involves injecting tracers around the tumor endoscopically.
  • This study demonstrates the feasibility of laparoscopic SLN in gastric cancer.

Observation:

  • A patient with early-stage gastric cancer underwent laparoscopic distal gastrectomy with SLN.
  • Dual tracers (technetium-99 and patent blue) were used for SLN identification.
  • Intraoperative endoscopy confirmed tracer injection and lymphatic basin identification.

Findings:

  • Pathology revealed gastric carcinoma with a micrometastasis in one sentinel lymph node (1/36).
  • The sentinel node was the only positive node identified.

  • The patient experienced an uneventful postoperative recovery.
  • Implications:

    • Sentinel node navigation with dual tracers is feasible for laparoscopic gastrectomy.
    • Standardization of SLN identification methods is crucial for accurate lymph node metastasis diagnosis.
    • This technique can enhance staging accuracy in gastric cancer surgery.
