Background:

Construction workers on the Øresund Link and Copenhagen Metro projects had high safety management levels.
Previous studies on the Great Belt Link construction indicated lower safety management.
Disability pension incidence is a key indicator of occupational health outcomes.

Purpose of the Study:

To evaluate the incidence of disability pension among workers in high-safety construction projects.
To compare disability pension rates between high and low safety management construction environments.
To assess the impact of improved safety management on long-term worker health.

Main Methods:

A cohort of 1,808 construction workers was followed for disability retirement.
Age-standardized incidence ratios (SIRs) were calculated for disability retirement.
SIRs were compared between workers on the Øresund Link/Copenhagen Metro and the Great Belt Link.

Main Results:

The SIR for disability retirement was 2.22 among study subjects (Øresund Link/Metro).
The SIR for disability retirement was 2.29 among Great Belt Link workers.
No significant difference in disability retirement rates was observed between the two groups.

Conclusions:

Substantial improvements in safety management did not lead to a reduction in disability pension incidence.
Occupational health risks, indicated by disability retirement, remain high in major construction projects.
Further research is needed to identify factors influencing disability pension beyond safety management.
