Detecting tampering with the record: a nurse expert witness's perspective

At times the health care professionals associated with a poor patient outcome will try to add, delete or change information documented at the time of the incident. In many cases these changes are made in ignorance of the devastating impact of tampering. The expert witness is often in a position to identify these traces of panicky behavior.
