The timing of micturating cystourethrogram (MCUG) after infantile febrile urinary tract infection (UTI) does not impact vesicoureteric reflux (VUR) detection. Performing MCUG sooner, once inflammation subsides, is recommended for infants with UTI.
Area of Science:
- Pediatric Nephrology
- Urology
- Infectious Diseases
Background:
- Infantile febrile urinary tract infection (UTI) diagnosis and management remain debated.
- Vesicoureteric reflux (VUR) evaluation is crucial in infants post-UTI.
- Current guidelines often recommend delaying micturating cystourethrogram (MCUG) for 4 weeks post-UTI to prevent false positives.
Purpose of the Study:
- To investigate the impact of MCUG timing on VUR detection in infants following a first febrile UTI.
- To determine if early MCUG influences VUR prevalence or grade.
- To provide evidence-based recommendations for optimal MCUG timing in pediatric UTI management.
Main Methods:
- Prospective data collection and case note review of 427 infants with first febrile UTI at a tertiary pediatric center.
- Infants were categorized into two groups: Group A (MCUG <4 weeks post-UTI) and Group B (MCUG ≥8 weeks post-UTI).
- Comparison of VUR detection rates and severity between the two groups.
Main Results:
- VUR was detected in 33% of infants in Group A (early MCUG).
- VUR was detected in 24% of infants in Group B (delayed MCUG).
- The difference in VUR prevalence and grade between early and delayed MCUG was not statistically significant.
Conclusions:
- The timing of MCUG following a first febrile UTI does not influence the detection of VUR in infants.
- Early performance of MCUG, once inflammation has resolved, is suggested.
- This finding challenges the necessity of a prolonged waiting period for VUR assessment after pediatric UTI.

The Micturition Reflex
The process begins with bladder filling, where the bladder wall stretches as urine accumulates. This stretching activates the urine storage reflex, mediated by the sacral spinal segments and the pontine storage center. Efferent sympathetic impulses stimulate the detrusor muscle to relax and the internal urethral sphincter to contract, facilitating urine...
