    • Patients with C5 palsy showed significantly narrower C5 intervertebral foramina on CT.
    • A larger C5 superior articular process and greater C4-5 spinal cord posterior shift on MRI were observed in the C5 palsy group.

    Conclusions:

    • Narrowed C5 intervertebral foramina and specific anatomical features are associated with post-operative C5 palsy.
    • This is the first study to directly correlate C5 nerve root impairment with C5 palsy.
    • Early foraminotomy and careful laminoplasty technique may prevent C5 palsy.
