Summary

A pulmonary infiltrate in a 61-year-old woman was diagnosed as Cryptococcus neoformans. Treatment with Amphotericin B and Ketoconazole led to a complete cure, marking a first for ambulatory Amphotericin B use in Panama.

  • Transbronchial biopsy and brushings confirmed Cryptococcus neoformans infection.
  • No central nervous system, immunological, or neoplastic involvement was detected.
  • The patient achieved a complete cure with Amphotericin B and Ketoconazole.

Implications:

  • This case highlights the importance of considering fungal infections in pulmonary infiltrates.
  • Successful ambulatory treatment with Amphotericin B was demonstrated.
  • This represents a novel approach to fungal infection management in the region.
